To Schedule a Biometric Screening Appointment:

1. In your browser window, type in: Ascendtowholeness.org
2. Click on “member” along the top bar
3. Scroll down to the left column menu and click “Biometric Screenings.”
4. Go to, or click on: www.screeningreg.com
   Login: ascend (all lowercase)
   Password: healthy (all lowercase)
5. Read the instructions and then scroll down to the bottom of the page and click, “Create a New Account.”
6. Enter the required information to create a new account.
7. Scroll down and click “Create New Appointment.”
8. Scroll through the list of available dates and locations. Next to the location and date of your choice, click the radial button on the right labeled: Biometric Health Screening. Then scroll down to the bottom of the page and click, “Create a New Appointment.”
9. Next to “Appointment Times:” click on the arrow to navigate up or down to select an appointment time.
10. Click “Save New Appointment”

You are done!

Alternative Biometric Screening Options

If you are not able to come during camp meeting the alternate methods are either to visit a LabCorp location or to have your physician complete the process.  The linked forms include the instructions about how to complete the process and use the form.  Please read those instructions carefully for the method that you choose.

• LabCorp – To find a LabCorp in your area you can visit www.labcorp.com.  On the LabCorp home page in the “Labs & Appointments” box enter your address or ZIP code.  Make sure to select the “Routine blood work” option from the “Select Service” drop down and click the blue “GO” button.  This will bring up the closest locations.  I would select the 100-mile radius to get a full picture of any in your area.

  The cost of a LabCorp visit will be billed to the employer by the Plan, so like the onsite event it is no cost to you.  Keep in mind this option may be more time consuming as it includes a venipuncture (blood draw rather than the finger prick method), and you will have to report your own blood pressure, height and weight.  (This option is potentially the best alternative to the onsite screening due to the low fixed cost and no financial responsibility by the Plan member). *Make sure to print and take with you the LabCorp form for your screening <click here=""></click>  
• Physician office visit - the cost of the physician visit and any labwork will be the responsibility of the employee.  The physician may charge you a copay (usually $25) and will typically charge the Plan for preventative care at a minimum.  The physician may have some of the measurements completed by a lab in some cases and there may be additional charges for either a copay or coinsurance that you will be responsible for. *Make sure to print and take with you the Dr Office Screening Form for your screening <click here=""></click>  If you go to your doctor, you are responsible for submitting this form to the health plan, using the information at the top of the form!
